We are an adult day service for adults in Portage and Wood County, WI that have a cognitive disability diagnosis. We encourage independence, community integration and emphasize a person's abilities. At 4U we work with individuals on what they wish to focus on and how they wish to do it! All of our staff encourage everyone to do and live as they choose but model a healthy and independent life style for our members! We are proud of the friendships that are formed between our staff and all of our members that attend - inside the service as well as out in the community!!
